Observation

The child with food allergies attended the center without a Individual Care Plan from 2/11/22-4/6/22. During this time there was not proper documentation of the child's individual dietary needs, including allergic reaction signs and symptoms and possible food substitutions. Dispute Description: There was no proper information relayed about any child’s allergies. Upon arrival we began auditing files and asked every parent to provide us with an Individual Health Plan (IHP) which this certain family didn't turn in until termination was mentioned. On this child's registration form (February 8, 2022) it is reported of only one allergy and Miss Alyssa ensured to follow that allergy upon arrival (March 21, 2022) before anything was communicated. Once the form was turned into us, we notified staff that he had an allergy and updated our allergy list once again on May 10, 2022. We also contacted the child’s mother on that day about filling out the fluid milk substitution form and medication authorization form for the Epipen. As well as requested, the child’s mother to provide us the Epipen in the original container with the pharmacy label. We noticed the Epipen was filled at the pharmacy on April 25, 2022. However, parent/guardian had failed to inform and update us about the child’s new medication. There was no proper information relayed about any child’s allergies.
